SB12 - Parent Consent Form

I hope this message finds you well. I am writing to share important information regarding Senate Bill 12 (SB 12), which requires parental consent before schools can provide health, counseling, and behavior support services to students.

As part of this law, school districts must have a signed parent consent form on file in order for students to receive services such as:

  • Counseling support for social, emotional, or behavioral needs

  • Health and wellness services provided by the school

  • Behavior interventions and supports designed to help students succeed

We understand these services are an important part of how we support the whole child and ensure every student is prepared to learn, grow, and thrive. Without a completed consent form, our students will not be able to access these supports if the need arises.

I've sent information regarding SB 12, along with a parent consent form and link with our families requesting they complete the form by September 12th.  Because this is critical to the services we are able to provide to our students, we will also send hard copies of the permission slip as needed. Parents can give consent or opt out by completing the form in skyward, by completing the google consent form, or by returning a hard copy. Below is the link to the consent form that was shared with parents.

SB12 Parent Consent Form (complete the link)

Thank you for your partnership in supporting our students. Together, we can ensure our children have every resource they need to be healthy, supported, and successful.
